By registering for, participating in, or permitting participation in any programme, class, event, or activity offered by Spartan Team Martial Arts, I, {name} acknowledge that I have read, understood, and voluntarily agree to the terms of this Waiver, Release of Liability, Assumption of Risk, and Image Release. I understand that participation in martial arts activities, including but not limited to training sessions, physical conditioning, drills, sparring, contact techniques, self‑defence practice, competitions, demonstrations, belt testing, special events, and the use of facilities or equipment, involves inherent risks that may result in injury. These risks include, but are not limited to, bruises, sprains, strains, muscle or joint injuries, falls, collisions, fractures, dislocations, and, in rare cases, serious or permanent injury or death. I voluntarily and knowingly assume all such risks, whether known or unknown, foreseeable or unforeseeable.

I represent and warrant that the participant (myself, or the minor for whom I am the parent or legal guardian) is physically and medically fit to participate in martial arts activities and that all relevant medical conditions, disabilities, allergies, or limitations have been disclosed to Spartan Team Martial Arts. I agree to promptly notify Spartan Team Martial Arts of any changes to the participant’s health or medical status. I understand that Spartan Team Martial Arts does not provide personal medical or accident insurance and that I am solely responsible for maintaining adequate health insurance coverage and for all medical, hospital, or related expenses incurred as a result of participation.

To the fullest extent permitted by the laws of the Province of Ontario and the laws of Canada, I hereby release, waive, discharge, and hold harmless Spartan Team Martial Arts, its owners, instructors, coaches, assistants, volunteers, staff, affiliates, facility owners, landlords, partners, insurers, and representatives from any and all claims, demands, actions, causes of action, damages, losses, liabilities, costs, or expenses of any kind arising out of or related to participation in activities, use of facilities or equipment, or presence on the premises, including claims arising from ordinary negligence, except where such release is prohibited by law, such as in cases of gross negligence or wilful misconduct.

I further agree to indemnify and hold harmless Spartan Team Martial Arts from any claims, actions, damages, losses, or expenses, including legal fees, brought by or on behalf of myself, the participant, or any third party as a result of participation in activities. In the event of an emergency, I authorize Spartan Team Martial Arts to obtain emergency medical treatment for the participant if I cannot be reached, and I accept full financial responsibility for any resulting medical care.

I agree to comply, and to ensure that the participant complies, with all rules, safety policies, codes of conduct, and instructions provided by Spartan Team Martial Arts and its instructors. I understand that failure to comply may result in suspension or removal from activities without refund. I acknowledge that this agreement applies to all current and future participation in Spartan Team Martial Arts programmes unless revoked in writing.

Image, Video, and Media Release: I hereby grant Spartan Team Martial Arts the irrevocable right and permission to photograph, record, and use images, video recordings, and audio recordings of the participant, in whole or in part, for lawful purposes, including but not limited to marketing, promotion, advertising, education, and communication. This includes use on social media platforms (such as Facebook, Instagram, and TikTok), websites, YouTube channels, printed materials, and other digital or promotional media, without compensation. I understand that these materials may be edited, combined with other content, and used worldwide for an indefinite period.

If the participant is under the age of eighteen (18), I confirm that I am the parent or legal guardian of the minor and that I am legally authorized to consent to this agreement on their behalf. I understand that acceptance of this waiver through the Gymdesk system, including electronic acknowledgement or checkbox confirmation, constitutes a legally binding agreement and has the same legal effect as a handwritten signature. This agreement shall be governed by and interpreted in accordance with the laws of the Province of Ontario and the laws of Canada applicable therein.

By completing registration and accepting this waiver, I confirm that I have read, understood, and voluntarily agreed to be bound by all of its terms.
